Application and Forms

The admission process for the 2016 - 2017 school year is now open.  St. Elizabeth School provides a rolling admission process; however space is limited per grade.  To ensure proper consideration, applications should be received as soon as possible.  Once your admission file is complete, letters of decision will be mailed out.

The following information is required to complete the admission file for Grades Pre-K through 8.  A list of these documents and the forms themselves are available through TADS student applications. 

Required documents for Preschool Junior Kindergarten/Kindergarten applicants are:

  • A copy of the student’s birth certificate
  • A copy of the student’s baptismal certificate
  • Pastoral recommendation form (one per family for Catholic families outside of St. Elizabeth Parish)
  • Non-refundable $225.00 Application Fee

Required documents for grades 1 to 8:

  • A copy of the student’s birth certificate
  • A copy of the student’s baptismal certificate
  • Pastoral recommendation (one per family for Catholic families outside of St. Elizabeth Parish)
  • Copies of transcripts (report card) for the current and previous year  
  • Copy of any Educational or Standardized Testing scores (including IEP, if applicable)
  • One confidential Teacher Recommendation
  • Non-refundable $225.00 Application Fee
